FORMS FOR EVENTS AND HIRES

Thinking of hiring a hall, foreshore, sports complex facilities in Break O'Day?  You would need to complete a hall hire form and a risk assessment form on each hire occasion.  It is also essential that you or your organisation have current public liability insurance.  A copy of same must be submitted to Break O'Day Council for our records.  If you do not have public liability insurance, complete the public liability insurance form and "purchase" this insurance for single daily use at a cost of $20.

If you are planning an event such as the regatta, a wedding, a party, an art event, you also need to complete an Event Application Form.
